Paying with a Superbill
If your insurance plan includes out-of-network nutrition benefits, you may be eligible for reimbursement.
After each self-pay appointment, we can provide you with a Superbill—an itemized receipt containing the billing codes your insurance company may require for reimbursement. You simply submit the Superbill to your insurance company following their preferred process.
Please note that reimbursement is never guaranteed and depends on your individual insurance plan.
How to Verify Your Benefits
Before your first appointment, we recommend calling the member services number on the back of your insurance card and asking:
-
Do I have out-of-network benefits for nutrition counseling?
-
Will you accept a Superbill for reimbursement?
-
Are CPT codes 97802 (Initial Nutrition Assessment) and 97803 (Follow-Up Nutrition Counseling) covered?
-
Is a physician referral required?
-
Is a medical diagnosis required for reimbursement?
-
What percentage of the visit will be reimbursed after my deductible (if applicable)?
-
How do I submit my Superbills (online, by mail, or through a member portal)?
Be sure to write down the date of your call, the representative's name, and the reference number for your conversation.
After Your Appointment
Once payment has been received, we'll provide your Superbill through your secure client portal. You can then submit it directly to your insurance company according to their instructions.
If you have questions about Superbills or insurance coverage, we're always happy to help point you in the right direction.
